NHS Scale and Polish: Free in Band 1 if Clinically Needed
A scale and polish is included in the NHS Band 1 £27.90 check-up only if your dentist judges it clinically necessary. Many patients pay nothing extra. Some do not get a polish at the NHS check-up and need to see a private hygienist for a cosmetic clean.

If your dentist decides a scale and polish is clinically needed during your Band 1 check-up, it is included at no extra cost. There is no separate fee, no top-up, and no upgrade to Band 2 because of the polish.
If the polish is not considered clinically necessary, the dentist may offer it as private treatment (£40-80 typical) or refer you to a hygienist.
The clinical-necessity rule in plain English
NHS regulations say a scale and polish is included in Band 1 only if there is a clinical reason for it. That means one of the following must apply:
- There is calculus (tartar) build-up contributing to gum inflammation or likely to lead to gum disease if not removed.
- You have a history of periodontal (gum) disease that requires regular professional cleaning to keep under control.
- The dentist has identified an oral hygiene concern that the polish forms part of treating.
If your gums are healthy, there is minimal tartar, and you have no relevant history, a routine cosmetic polish is not part of the Band 1 charge. The dentist can decline to do it under NHS terms but may offer it as private work at a separate cost.

What to ask before the appointment
If you would like a scale and polish at your NHS check-up, raise it at the start of the appointment. The dentist can tell you whether they consider it clinically necessary as part of your Band 1 examination.
If they say it is not clinically required but you would still like one, ask whether they can do it as a private treatment, what it would cost, and whether they can do it on the same visit or whether you need a separate hygienist appointment. Always agree the cost before the work starts.
Never let the practice present a polish to you as "part of the visit" without confirming whether they are billing it as Band 1 (NHS) or as a private add-on. If you are an exempt patient, only the NHS-included polish is free for you.
Who pays nothing for a scale and polish on the NHS
If you qualify for free NHS dental treatment, the Band 1 charge does not apply, so a clinically necessary polish at your check-up is free along with the rest of the visit. Exempt categories include children under 18, under-19s in qualifying full-time education, pregnant women and new mothers (with a Maternity Exemption Certificate), and people on qualifying benefits or holding an HC2 certificate.
In Scotland, NHS dental examinations are free for everyone. A scale and polish included in the exam carries no additional charge regardless of your benefits status.
